Have your ID, a recent bill or statement with your address, and any credit reports or collection letters nearby.
Allow about 10–15 minutes. This form does not save a draft if you close the page.
Six short steps. Only a few fields are required (marked *); fill in everything you can.
You'll need your full legal name, Social Security number, date of birth and current address. You'll also need your IdentityIQ login (or sign up during Step 5). Your SSN, birth date and IdentityIQ password are encrypted on your device before sending. Never enter bank account numbers or bank passwords.
This intake is not a contract and nothing is charged. You'll receive your written rights disclosure and agreement before any service begins.
